Confidentiality of the ASAP!® Adult Health Survey
The ASAP!® Adult Health Survey is brought to you by the Healthy Challenge Wellness Program, as a service for City of Fort Worth employees who are interested in learning more about what action steps they can take to promote better personal health. The website is maintained by the Healthy Challenge Wellness Program. Healthy Challenge guarantees the privacy of your information during collection and transmission to the processing office. Your individual responses to survey questions will remain confidential and will not be shared with your employer, health plan benefits department, or anyone else unless you give your express permission. Healthy Challenge uses a Secure Socket Layer (SSL) to encrypt all survey responses so that prying eyes cannot see the answers given by any individual. Raw data from any survey(s) you take may be used anonymously to produce aggregate reports that can be used to identify population needs for preventive health programs.
Healthy Challenge members can retrieve their results from previous surveys by providing their Employee ID Number and unique password.
